One of the things that makes Ashwood Support Services unique is that we specialise in providing support for those living with a mental health condition, including those with complex care needs or who have dual or multiple diagnosis. We understand that the type of support that will help one person will not be helpful for another, especially as a persons environment, social support and access to mental health services will play a large part in dictating what they need in their recovery and day to day supports.
For some, simply having regular check ins and a chat will be enough to keep them feeling well, looked after, connected and in control. For others, they may need supports in simply gaining access to the services they are entitled to, staying safe and keeping on track with eating regular meals or going out to get some vitamin D. Wherever a person might be at in their journey, we are strong believers that each person is the expert in their own recovery and what it takes to stay well, and so we make sure that the people we are supporting are central in developing any safety plans, recovery goals and future planning that may need to take place.
As a result of specialising in supporting people with mental health conditions, we are used to liaising with acute care wards in both the public and private health systems to ensure that upon discharge the transition from hospital to home is as seamless as possible. We have often been involved in care team meetings with psychiatrists, mental health nurses, social workers and other mental health professionals to make sure that everyone is on the same page when it comes time to discharge, and that we are able to have an ongoing dialogue with any treating providers the participant consents to us communicating with.
